1. What is the definition of the global "INBOX" directory.

A mix INBOX is a directory ~/INBOX as opposed to an mbx INBOX which is a file ~/INBOX

So, a user can not simultaneously have a mix INBOX and an mbx INBOX.

2. Does [the feature that messages from the mail spool are moved to the mbx INBOX] still hold [for mix]?

Yes.

3. Now, people can presumably create subfolders of "INBOX".

As much as I'd like to avoid that from happening, you're probably right. However, note that even though INBOX is case-independent, any submailbox of INBOX is not and can only be referenced by the all-upper form.

So is a folder path of "INBOX/blah" going to correctly use the right path for "INBOX" and then look in the appropriate subfolders? In that way, we end up with 2 folder trees -- INBOX and "friends" and wherever you keep your "imap folder path" folders for everything else.

The answer is supposed to be "yes". I don't think that it's a good idea to use submailboxes of INBOX, but some people will do it regardless of what I think.
