I was testing with an email address on a non-existent server. Does this mean
that "permanent failures" are generated only if the destination email server
responds with a failure message?
If so, does the POST Fail template receive notification on ALL delivery
failures, no matter how it happens?

If you configure notification of permanent failures then the template will
only be called if a permanent delivery failure hs occurred. also, whenever
you change settings you need to restart the POST Service.
BTW - a "permanent failure" is usually when the remote server responds with
a 5xx error upon the issuance of a RCPT (specification of a recipient).
