I think I have pinpointed the issue, Howie. 

The issue, I believe, is related to the postfailed.cfm file. Some custom
code previously written on our end was causing the mbx record to be deleted
in an untimely fashion, leaving the mail file to be undelivered. 

I will test this thoroughly tonight, but my initial tests suggest we have
resolved the problem. And, of course, it was unrelated to your software's
design.
