FusionMail, by default, requires SMTP authentication. I recommend using the iMS Relay server with applications like CFMAIL. It doesn't require smtp auth (but is still secure). I ususally recommend running it on a non-standard port like port 2525.
Turned on IMS Relay service, added my 2 IPS on my web server to allowed IP's (one mail one web server with CF), restarted, added port=2525 to CFMail tag and still NO LUCK.
If you want to use the iMS SMTP server instead then you can put the CF server into the allowed IPs of the SMTP Server and then, I believe, it should work. I requested that FusionMail v2.5 include support for the iMS allowedIPs setting but I'm not 100% certain that it does.

Why in the world would it ONLY work if I send a local copy to a BCC or CC, for any external email in the TO ?
I would use CFX_iMSMail but I have so many CFMail tags to change...
