Re possible hardware failure: If you're measuring 5V on the 5V rail while powering from Vin and nothing on the board gets hot, most likely your hardware is just fine. The warning on the Seeed page is a pretty rare failure mode and has been observed only on IOIO-OTG AFAIK. You're using the IOIO v1 I figure?
Assuming that's the case, what's next? If you're getting the ADB icon on the Android, then your IOIO is alive and kicking and is able to establish a connection with the Android. In that case, what Android version are you running? From Android 4.2.2 and onward, the IOIO no longer works over ADB, but only over OpenAccessory, which requires you to actually turn ADB *off* on your Android. It also requires firmware of version 3.10, so if you have an ancient firmware version you'll need to upgrade. Unfortunately, for that you'll need to find an Android of version 4.0 or lower, as IOIO Manager compatibility has been broken since 4.1 or so. Note that this issue have been addressed on the IOIO-OTG and does not exist there (firmware upgrades are done from a PC).
