It is true that nothing gets hot to the touch. I also measured the 3v3 rail 
and it is looking good. 

Regarding the firmware upgrade, IOIOManager doesn't see the IOIO, so I'm 
not sure that is an option. 

I'm running much older versions of Android that don't support OpenAccessory 
(2.3.6), so I require ADB. I did try turning that off and then on again, 
but there was no change. What kind of scenarios can you imagine that would 
cause the Debug icon to show up, but not allow the IOIO to connect? My 
thoughts were that the PIC firmware was gone, or the bootloader was stuck, 
but in some other posts I see you mention the Stat LED staying lit (vs mine 
staying dark) in those cases. 

The debug icon indicates that the PIC firmware is somewhat working, right?
