Yes, this is a known failure mode that has been identified and warned of in
the past. A fully charged LiPo battery is about 12.6V and in conjunction
with a sudden current spike resulting from connecting the tablet can cause
the 5V regulator to fail.
There is an upcoming hardware revision that fixes this issue. In the
meantime the only way to guarantee this doesn't happen is to not exceed 10V
on the input or use Android devices that have been tested to having
sufficiently small inrush current.
