Is there anything at all connected to pin 1, which might pull the pin low
sometimes? Is there anything that might be pulling the boot pin low
sometimes (causing the IOIO to boot into bootloader mode)? Other than that,
I don't think there's much we can do. Well-behaved IOIO boards do not
behave like you describe.
