That was it! I had to delete all the existing (stale?) "iPhone USB #" entries, then create a vanilla 'iPhone USB' entry. After that, when I connect the iPhone via USB cable and choose 'USB only', it takes about 7 or 8 seconds to set up the network interface, but it does work, without futzing with wifi or bluetooth.
