Fixed — the registries and the retired flag moved into a
`shared_ptr<ConnectionState>`, and every continuation and interrupt handler
captures a `weak_ptr` to it instead of `this`. Covered by
`failedWriteAfterDispatcherDestroyedIsIgnored` and the fetch variant, built as
you suggested.
