I think I know the root cause now: assign region out of SSH. A region server is
dead, before SSH handles it and completes the log splitting, a CM assigns a
dead region. That's why it doesn't happen all the time. Fortunately, that's
not a scenario normally happens to a real cluster.
Let me check if we can prevent from assigning a dead region out of SSH from the
master.
