Patch v4 changed the default prefetched result size cap from 512M to 256M;
fixed a the prefetched result size counter leakage in closeScanner(); added
some tests to make sure the cap is followed, i.e. prefetching only if the cap
is not met; and added some stuff to make the new test possible.
