Fixed in 76a8ea0. Provider functions and factories now use a settings
snapshot taken under the reload lock, including embeddings and rerankers.
Client construction and embedding calls run after releasing the lock. Added
concurrent reload tests for all nine entry points, plus checks for delayed
construction and refresh during dimension detection.
