Yep. That's what I mean... We need to solve the "let's start the scan exactly 
here" problem in any case. It does not have to be server-stateful.
So I propose we solve the "let's start here problem" first (OFFSET), then use 
that to implement CURSORS.

CURSOR is simple to use, OFFSET is more general. We should do both and build 
one on the other.
