If you can read PDF syntax, it's easy to find the coordinates,
however: to do this programmatically, especially when the document gets
more complex, IS VERY DIFFICULT, IF NOT IMPOSSIBLE!!!

You won't find any simple example, because what you are asking is one of
the most complex issues in PDF.

My advice: tell the person who asked you to do this job, that the given
PDF document IS NOT A TEMPLATE!!! Ask him to provide a PDF template that
contains form fields (preferably in AcroForm format).
