The checkboxes are not opaque. The color is in the form field and will
display if I edit it in Acrobat.
We do not have a support contract for iText ate this time, but I have
requested it. But I do own the book!

Does anyone have example java code for recreating checkboxes on an
existing PDF file so that updates to background color will display.
It's possible if the appearances of the existing checkboxes aren't opaque,

I guess it's about an hour work to write an example. Do you have a license
/ support contract for iText?
