Now I've checked with a file created using Open Office, and I can reproduce it. This indicates that Open Office puts something in that PDF file that causes this problem. I don't experience that problem if I create the form using iText or Acrobat. That's the reason why you didn't get an answer: we weren't aware of the problem because it's not an iText problem.
