I've found the culprit.
OOo adds /NeedAppearances true
iText flattens the file, removes some stuff from the /AcroForm 
dictionary, such as /XFA, /SigFlags, but not the /NeedAppearances key. 
Once this key is removed, the problem is gone.

I think Adobe Reader X is correct: it discovers that appearances are 
needed, it applies some changes (regardless of the fact if there are 
fields that need to be rendered; it ignores the fact that the /Fields 
array is empty) and it stores the file with a compressed xref table, 
improved info dictionary, no more unused objects,...

I've updated iText so that the /NeedAppearances entry is removed when 
flattening a form.
