The problem is that this is an XFA-based PDF (aka dynamic PDF).

As such, you can only sign this using either standard XFA Signatures or,
if you require PAdES, then PAdES Part 5 (not parts 2-4).  Neither of these
types of signatures are supported by iText at this time.
