Sorry that it took me that long to answer.

I just got the green marks without much trouble, both in Adobe Acrobat 9.5.5
and in Adobe Reader XI 11.0.3.

In case of your latest document,

I only needed to trust the root certificate in the viewer. I tried it both
ways:

1. Using "Manage Trusted Identities" (or so I assume as I'm using the
software with German localization) I added the root certificate (
RootCA5_DER.cer
<http://itext-general.2136553.n4.nabble.com/file/n4658540/RootCA5_DER.cer> 
) to the certificates known to Adobe Acrobat / Reader and checked the
checkbox for the use of the certificate as trusted root.

2. Using the certificate details view from the signature properties dialog I
selected the root certificate and chose to trust it. (This was only possible
because you included the whole certificate chain in the signature
container.)

Either way I got:

In case of your older documents, e.g.

I additionally had to add the intermediate certificate ( OperCA5QES_DER.cer

) using "Manage Trusted Identities", but ONLY add, no need to set any
special trust.

After adding the certificates to the Adobe Reader store, have you checked
the "trusted root" checkbox?
