Remove ivtv with
modprobe -r ivtv
then add it again,
modprobe ivtv
Then post everything that goes to logs (dmesg) when the driver is
loaded. For example, the above missing stuff looks like the firmware was
not loaded, but it is impossible to say without you posting all the
debug logs.
