Nick,

Running 'modinfo ivtv' shows (among other things) that also:

parm:           ivtv_first_minor:Set minor assigned to first card (int)

In the case of the cx18 driver:

otherbox ~ # modprobe cx18 cx18_first_minor=3
otherbox ~ # ls /dev/v4l/
vbi3  video27  video3  video35
