^^^ these are not needed (with the possible exception of i2c_algo_bit and
videodev). The others are loaded automatically.

Try 0.3.7g. The tuners should be back :-)

Don't do this, it's not needed.

Are you sure this is the correct frequency? The picture looks like the
frequency was almost right but not quite. If you know the exact frequency
(e.g. Nederland 1 = 203.25 MHz then set the frequency with 16*203.25:
ivtvctl -f 3252.

Otherwise I see nothing strange in your log.
