Mark, Looks like you are building ivtv dirvers against the correct kernel sources. My recommendation at this point would be to do a clean build/install of the kernel and kernel modules and then to simply do a fresh build/install of the ivtv drivers w/out overlaying the kernel modules. It looks like you've got the firmware in right place/places.
