Coming from Gentoo-land, I config all my kernel's via
'make menuconfig' and build all my kernels from
source.  

Just to let you know, I am not too familiar with
installing/building kernels under Fedora.  But yes, I
would recommend building your 2.6.16 kernel from
source and it really does not matter which one you
boot from when you do this.  

If you want to make life a little simpler, you can
copy the old '.config' from the old kernel src dir you
boot from into the new kernel source dir and do a
'make oldconfig'.  This will ask you questions on the
command-line about all new additions in the new kernel
to help you bring the '.config' up-to-date.  Then
simply go back in with 'make menuconfig' and set the
kernel drivers/modules you need.

Then do whatever you need to install the kernel and
modules.  In my case with Gentoo, I simply do 'make &&
make modules_install && make install' and its all done
for me.

As far as the ivtv drivers, I believe building with
'make clean && make && make install' should clean out
all the old build files and give you a clean ivtv
driver installation.
