The above 2 lines is your problem. You seem to have a unknown tuner type 
for the kernel. You can try and set it 38 in /etc/modprobe.conf by adding:

   options ivtv tuner=38

Now reload the ivtv module which should make the tv tuner work but the 
radio will probably not.

Hans, a potential tester with a FM1216ME MK5 tuner in his PVR-350 :-)
