You do not need the patch mentioned below as your firmware is loading.
The bug was fixed in later 2.6.20 kernels (I thought 2.6.20.2).

Does your kernel log have any error messages when you try to use the
card? Try to use the command line for test and not mythtv, does the
below work?
 ivtv-tune -c <channel> or ivtv-tune -f <freq>
 cat /dev/videoX > test.mpg
