I have very similar issues on a fedora box.  I still have the issues and
haven't resolved them, though I've figured out a few steps to mitigate
them a bit.

1) I use an older kernel (.20) with the 0.10.6 ivtv drivers.  These seem
   to be *slightly* better than the newer (1.0.3+) or bleeding edge
   drivers.
2) I do set the latency timer of my drive higher
3) I also set the look ahead cache higher (hdparm -a).  I doubt this
   helps though.
4) I have the mysql buffers multiplied by about 4 (/etc/my.conf)
5) I use ivtvctl to turn on debugging:

   /usr/local/bin/ivtvctl -D585

   With this on, and writing a *ton* of data to the drive it never
   crashes.  If I leave debugging on and filter out the data via rsyslog
   so it doesn't go to the drive, ivtv times-out usually when doing
   something else on the front end.

I think what I really need to do is buy a better motherboard.  I have a
fairly cheap one with via chipsets.  Is yours a via chipset by chance
("lsmod | grep via" is the quick way to tell).  I got my motherboard as
a cheap deal at Fry's and, well, it sucks.

I also have a silent PCI graphics card that I should probably replace
with one in the ePCI slot instead.  Finally, I've heard having a second
hard disk for the system vs video recording may help.  But I'm betting
on the motherboard being the real problem.  I won't know, of course,
till I replace it.
