I think you're going to find distro differences here.  I run a mix of 
Debian versions and see:

etch: /usr/include/linux/usb_ch9.h (in package linux-kernel-dev)
sid:  /usr/include/linux/usb/ch9.h (in package linux-libc-dev)

(they look the same at first glance but note the '_' and corresponding '/').

I don't know about other distros.  Maybe it's just Debian.
