Hi By design , if you press the keys for say word twice quickly, JAWS will spell the word. Then if you next use JAWSKey+right or left arrow, JAWS remains in spelling mode and will spell each word you move to. To get back to regular Say Word mode, press the keys for say word once. As far as JAWS losing focus, make sure that the application window you use is maximized by pressing Alt+space and then press x. In some applications, this problem is more prevalent, and the only solution that I've heard of is to do what you are doing by using Alt+Tab.
