It's a very simple form, and FS only wants to know why you want the dongle,
and some basic information about you. Nothing very personal. I have one, and
it was very easy to get. I just told them that I traveled around a lot and
did not want to haul a computer around with me. I also was having to
reinstall JAWS often at that time, and FS did not like me requesting
Activations. The dongle will cost you $45, but I like having it. Now, I can
just plug it into any computer, and depending on who owns the computer,
either install the correct version of the program for their PC, or use the
latest portable version, which still requires the installation of the video
intercept on the computer, but this can be removed after use. One thing that
I don't like about it is that I have two laptops, and sometimes I want to
use JAWS on both of them at the same time. I am not sure if I can get
another one for $45, but I doubt it. Another thing, FS has the dongle rigged
up so that it will not appear in the list of drives in File Explorer or My
Computer. So, forget trying to make another copy. LOL.

I purchased my SMA's the other day for 16 & 17 and tried to purchase a
Donggle. However, when I inquired about the Donggle I was told that I had to
fill-out a form and get approved for the purchase. What is this form the
sales person speaks of?. He said that he would send it to me in an e-mail,
but I never received it. I wanted to inquire here about what it is before I
start trying to follow up on it. Any help with this will be greatly
appreciated.
