I've never had a problem closing Narrator in years of using it. On the Narrator dialog (_not_ the Utility Manager dialog) simply TAB to Exit and press ENTER. If this doesn't work for you, then your Windows installation may be corrupted. Also, you must close the Utility Manager separately, which I do by pressing SHIFT+TAB to get to the Cancel button and press ENTER.
