[Max] This assumption is false.  These days, ISPs like us don't have their
own nationwide pop network, it just doesn't make sense.  So we host our
datacenters at places like Exodus, which most definitely do have
per-megabyte-per-second charges.  I'd be surprised to find an ISP that
doesn't have a cost-per-megabyte-per-second, unless Enron is doing their
accounting.
