myisamchk came back clean.  I checked the 'alarms' table to see what was entered for the unacknowledged event:

        "id","date_start","date_stop","interface","type","active","referer_start","referer_stop","triggered"
        75812, '2006-02-03 10:00:12', '0000-00-00 00:00:00', 15271, 38, 6, 129933, 0, 1

I also checked the debug/logging option and found the following entry for the up event:

        10:00:16  :  H   5 :  I 15271 :  P  20 : app_status(): running;1 Instance(s) -> alarm(38): Event Added: 182379 (time P:0.12 | 5.28)

Yet the entry never clears from the tables.  Is there anything else that it uses to match an alarm state?  Or where would I look in the specific php file to debug the issue?

Try to find any corruption on your alarms or events table with myisamchk

An event become the UP event of a Down alarm when the Event Type and Interface
ID matches.
