I also have a similar phenomenon with %d in the tab title. But instead of
stopping to update, it just produces no/empty output. When I open a new tab
with "%d : %n" set as title, it shows the current directory for a few seconds
and then there is no output for the directory anymore. I.e. "~ : bash" becomes
": bash" short after opening a new tab.

Using "%D : %n" instead works, the directory does not disappear from the tab
title then. But the output of %D bloats the title with information I don't
need.

It may seem a subtle problem but it's making me think about switching my
terminal emulator.
