Do you mean changes to the module itself, or do you mean changes to the 
footprint name in EESchema in order to assign a different module?   If 
you mean the latter, PCBNew will certainly report that the module has 
been changed even if a .CMP file is not present.   However, I do find 
the former a bit problematic; on one of my boards I changed the silk on 
the 0805 footprint, and not being able to (quickly) find a means of 
updating all the 0805 components already on the board I resorted to 
editing the PCBNew file in a text editor (which was quick and easy, but 
if there isn't a better way there ought to be).
