HMM ... I have Adobe Acrobat Reader v 7.0.1 and I cannot fill out the
form I have. I checked under the About Adobe Plugins and it claims to
have the Forms plugin installed.

When I go to save the original form (off the web site) I get a message that
says "This document does not allow you to save any changes unless you
are using Adobe Acrobat 7.0 Standard or Professional ... etc "

I guess I don't have quite the right version.

I will check out Adobe and see if I have to buy something.
