Here is Joe's post from 2001 (I hate to throw anything away).
For anyone interested in making their own set of adaptors, go to you local
electronics store and buy (1) DB25 MALE to RJ45 and (1) DB9 FEMALE to RJ45
($2-3 ea)
Pin them out as below and just add a standard straight through 8 conductor
cord as needed. I've used up to 50' non-cat5 cords and have had no problem.
This setup works for KSU's and voicemails. ('cept for the D308, which is
9pin from the KSU). Just remember to set flow control to NONE in
Hyperterminal (or your com program of choice).

I thought someone listed a recommended pinout for the rj45/db adapters
to use between a pc and the kxtd and tvs several years ago on this
list. there was even someone selling kits. I just spent 2 hours
searching the archives and couldn't find it.
Anyone remember it and can post it? even Michaels /before page
doesn't show it anymore. i tasked a tech to build a bunch of these and
would like to stick to any informal standard. i was looking at the
cisco and adtran admin connectors, but they only need four pins. i
don't need the db9 to db25 pin out, have that down solid ;-)
i'm tending toward just using 2 to 2 thru 8 to 8 and then use 1 for 20
on the rj45 plug.
could dig up the old dec plug pinout, it was unique in that pin 3 and 4
(of 6) was "ground". the short made sure accidentally plugging into
phone jack couldn't put 48 vdc on the rs232 terminal pns. pin 2 and 5
were rx /tx, 1 and 6 were dsr/dtr, cable was crimped 1 to 1. but
crimping 1 to 6 (flip one end of cable over, like regular line cord)
made a crossover cable to test two terminals back/to/back. might be a
good idea, put cts/rts on pin 1 and 8 of an 8 pin rj45, hey i like it.
