Bob, One thing to check is the rollover or hunt capability. I've seen where calls were rolling over improperly or to a number that was not even present in their list. Therefore only intermittent calls would hit that number.
Their VOIP provider could also have a similar error, where a wrong number was put into forward to if the others were busy. There could be some other causes, hard to make suggestions without being there to check it out in person, such as a bad lightning protector locking up after a ring. I had a customer who did not realize they had voicemail service from the phone company on one of their lines. I would pick up after longer ring time than their TVS voicemail so it only went to that when all the lines were busy.
