Normal routing is to Line 1 first then 2,3,4.
I had the customer unplug the sets one at a time while the line 4 was lit
and NO extension being unplugged caused the light to go out on line 4. He
then tried it with only one set at a time plugged in and had the same
results. I am leaning toward a bad expansion board or base board in the
KSU.

It appears that something within the system is locking up line 4.
As Jim mentioned, a bad station could auto answer.
I have not seen it with cordless, but I have seen it a number of times
with the analog sets.
When the line is lit, you could go through and unplug each station at
the ksu and see if the line goes out when you hit a certain extension.
I had it happen at a restaurant where the offending phone was one that
was forgotten about in a back storage room, and had crusty old wires
too.
It could also happen from just corroded extension wiring, or something
in the system too.
You could temporarily program a phone's DSS buttons to show groups of
extensions (even unused ones) and see if one of them lights up when
the call comes in.
