I had a similar situation awhile ago. It turned out to be the dialing parameter setting. Someone had removed the default 1xx and 2xx setting. This caused the VM lights not to light. Like you, I also saw the VM send out the code to illuminate the VM light with luck.

When I got there, it was hooked up with two sets of wires to two ports on the KXT so I rewired it using only one KXT port. I checked all the settings and made a few changes but finally gave up in frustration.
