yes it is a bit tricky to get it working (e.g. I haven't check it but in the last screenshot of the l4dmaps shovel the arm animation doesn't look correct). In the month after we create the first version of your model I helped Todd to create the foamfinger for "Suicide Blitz 2". During this process we detected few smaller issues and I have had to adapt the qc file. As far as I know the foamfinger works without any problems now.
After that I have send you an updated version of your model (4.June.2011).
I use the Softimage Modtool and can't help with the 3ds Max import/export problem but I thought you were at least able to compile it by your own. Let me know if there is still a problem or you want to have a compiled version.

bunny had more luck than I - I never personally got it working on my own
system in 1st person due to compiling and export issues.

He laid out the process pretty well in the previous message. How I started
was by decompiling a weapon that was close to mine in terms of shape and
size.  I opened it in 3ds Max, replaced their weapon with mine, skinned my
weapon to their bones, and exported.  I could never get it to compile
properly, and bunny said he got it working, but I never saw it in action
(but I would love to).

To create your own custom melee weapon you need some experience in
creation/compiling of custom prop for the source engine.

- you need a custom melee weapon script in the vpk folder "scripts/melee"
e.g. shovel.txt
- you have to add the custom melee weapon to your mission file in the vpk

campaign e.g. "meleeweapons" "fireaxe;crowbar;cricket_bat;**
katana;baseball_bat;knife;**shovel"
- to spawn the melee weapon in your campaign use a "weapon_melee_spawn" in
Hammer and set "Melee Weapon" value = shovel and browse for the "World
Model" value to the w_ model file e.g. w_shovel.mdl
If you run your campaign the custom melee weapon should work.
The only limitation is that it is not possible to use a custom hud icon
for the melee weapon.

I recommend to use a "true" custom melee weapon with a custom name. A
replacement melee weapon of a valve melee weapon may cause trouble.

 Last time i heard someone doing something like this, it required editing
some official weapons files or something and then packing those files with
your campaign in order to get them work.

However, some campaign that i played and used such method, it swapped
some weapon color to another as long as i  had that campaign installed.

Valve doesn't really support such things officially and so
bubblegum/ironwire/etc is needed to make such things work.
