hledger calls it the description. Narration is a traditional name for it (http://en.wikipedia.org/wiki/General_journal). It's where you describe the transaction in as much detail as necessary.

For me sometimes it contains a payee name, sometimes not. Now and then an explicit payee field would be handy. I've started an informal convention of putting the payee at the start, with two spaces separating it from the following description if needed. Two+ spaces is a sort of de facto *ledger field delimiter, and not too obtrusive.
