Well I usually know what I'm doing, but that doesn't necessarily mean
other people agree with what I do!  And I can't give you an example that
matches yours, because I haven't had to deal with that type of document,
but here is one of my citations, created with SourceWriter, for a US
Census:

1880 U.S. census, Kings County, New York population schedule, Brooklyn,
New York, enumeration district (ED) 1014, p. 320D, dwelling 244, family
321, household of Arthur H Beckwith; digital images, Family

and here's one for a Passenger List found on a website:
