Is it possible that you are seeing the 'subsequent citation' form of the source, in the cases where details are missing? In my experiments last year, I found that the implementation of subsequent citations, for some templates, dropped certain detail fields that really should be kept (or, at least should be kept if the value had changed since the previous citation).
A work around for this design defect is often to fudge the template by placing data in a different field than what is suggested by the template.
