Fair enough. I guess for my taste it seems a little odd to start the narrative with a name that hasn't been introduced yet. So I could do without 1) if 3) were presented before the nickname is first used in the narrative. If that is not possible, then I would prefer the option stated -- to include 1) and omit 3) altogether.
As an aside, I see another advantage to using 1). In families where there are a lot of people with the same name, highlighting the nickname is helpful for the readers who only skim quickly through the report.
