I have found this to be very useful.
                 "Control Panel"
                 "Printers and Faxes"
                 "File"
                 "Server Properties"
                 "Create a new Form"
                 input a new form name in "Form name"
                 enter the required paper size in "Width" and "Height"
                 use the  "Save Form" button.

Now any printer including "PDF printers" which has a paper size of
"Custom" should have the New Form you have just created.
