I would use the newest version of NC State Board of Health and then note that
other names such as NC Department of Health and NC Department of Health and
Human Services have also been used in the past.

In my family file I have 17 death certificates all from Ancestry.com but I am
confused how to cite them. They are all from North Carolina but some have the
State Board of Health header and some have
NC Department of Health and NC Department of Health and Human Services Human
Services header I am guessing they changed it a while back. Should I use one
citation or should use two?
