I'm coming in a little late on this topic but here's my 2 cents worth.

First, ask yourself "will someone else be able to reproduce my work following my
sources?" If the anwer is "yes", don't beat yourself up too badly about "proper"
format. Second, don't drive yourself crazy with tiny details.

Having said that, I use just one citation. Ohio has also changed the official
name of the Department of Health several times over the years. I just use Ohio
Deparment of Health. No matter how you Google it you can still find it :-) So,
for your source, I would just use North Carolina Department of Health. And I
would spell out North Carolina for those who do not live in the United States,
even if the "official" name uses NC. They may not realize that NC is the US
Postal Service abbreviation for North Carolina. And, of course, I would use the
template for digital images with Ancestry.com as the repository.

The only time I change sources is when the repository and format change. Some of
my death certificates are digital images from FamilySearch, some are
non-certified copies from the Ohio Historical Society, and some are copies from
the Ohio Department of Health - certified and non-certified. So I currently have
3 distinct sources for Ohio death certificates.

In my family file I have 17 death certificates all from Ancestry.com but I am
confused how to cite them. They are all from North Carolina but some have the
State Board of Health header and some have
NC Department of Health and NC Department of Health and Human Services Human
Services header I am guessing they changed it a while back. Should I use one
citation or should use two?
