This is the way that I do it.  This is the format that FamilySearch uses,
which is why most programs consider this a 'standard' way of recording
locations.  It doesn't always apply well to other countries and people who
have no need to sync with FamilySearch data don't always want to 'conform'
to this format.  To me, it's always good to have a 'standard' that is
recognizable and inserting the commas to hold the place of city or county
identifies which information we are missing and it's easy to see.  Entering
"County" following a county name where there is no city is extremely
helpful to the user so they know whether it is a city or county; how else
could you know or how would another person viewing your database know
that?  But if you have the different fields, it is usually understandable
by other researchers at least.  Personally, commas or not, I think County
is a very useful thing to get used to adding when you don't have a city,
but I'm not good at remembering that :-)

The United States vs USA is the same issue -- FamilySearch changed the way
they record that several years ago; they used to use USA.  My understanding
is that using abbreviations like that, it is still possible for someone to
misunderstand and perhaps there is another area with those initials.  It
seems impossible to us but if United States is more descriptive then that's
fine with me.  And Legacy makes it SO simple to change all of your
locations to be standardized.
